Instead of billing insurance, members pay an affordable monthly fee that covers primary care services including routine visits, sick visits, wellness management, and direct access to your provider. How Much Does it Cost?

Initial one-time sign up 

Adult Membership

$80/month per adult (no hormone replacement therapy)

$95/month per adult including hormone replacement therapy

Family Plan — $225/month
Includes 2 adults (same household) + 2 children ages 13–18
Each additional child — $25/month

Couple Discount: 5% off total monthly cost
One time Enrollment Fee which includes your first visit: $150 per person or $250 per family
